The erasure of Autogynephilia

I have previously written about Autogynephilia, and developed a chronology of its major events and persons up to 2010. 

The Autogynephilia syndrome and meme was a major brouhaha in the early 2000s, especially after Bailey’s The Man Who Would Be Queen was published in 2003. While many dismissed the concept as pseudo-science, it came to dominate discussions of typology of trans women to the extent that alternate discussions of such typologies were drowned out. While there is an intuitive difference between early transitioners and those who first became husbands and fathers before transitioning, the model as laid out by Freund, then Blanchard and then Bailey aligns each type with a sexual orientation (a mistake earlier made by Harry Benjamin), erases late transition androphiles and early transition gynephiles, insists on referring to heterosexual trans women as ‘homosexual’ and defines the gynephilic late transitioners as ‘fetishistic’ (again a mistake made by Harry Benjamin). The Clarke Institute (later CAMH) in Toronto is especially identified with the syndrome as Freund and Blanchard had top positions there. And, like it or not, the history of trans in North America in this century cannot be told without reference to the debate over Autogynephilia.

However there are writers, both trans and cis, who leave out what we would expect here, even though many of them quietly put Bailey’s book in their bibliography or further reading while saying nothing about it in the text.

This article discusses writings that nevertheless:

  1. distinguish two types of trans women, such that the reader would expect a discussion of how this is similar to or different from the concept of Autogynephilia – but this is not discussed;
  2. discuss the Clarke Instritute/CAMH, particularly the GIC, but say not a single word about Blanchard or Autogynephilia;
  3. retell the histories of US/Canadian trans persons and politics in the 21st century but say not a single word about Blanchard or Autogynephilia.

1985. 

Betty W. Steiner (ed) Gender Dysphoria: Development, Research, Management. The first book from the Clarke Institute of Psychiatry in Toronto. The full model HSTS-Autogynephilia is explained by Kurt Freund, although at this stage without the word ‘autogynephilia’.

1989

Ray Blanchard. “The concept of autogynephilia and the typology of male gender dysphoria”. The Journal of Nervous and Mental Disease, 177, 616-623. 

1990

Kurt Freund retires, and Ray Blanchard becomes Head of Clinical Sexology Services at the Clarke Institute of Psychiatry. 

Viviane Namaste wrote a report “Access Denied” funded by Health Canada to do needs assessment for trans persons in Toronto. Of her sample of 33, 19 were enrolled at the Clarke GIC. The trans academic also visited the Clarke and spoke to some of the staff. There is no mention of Blanchard, Autogynephila or the 1985 book.

1996

Kurt Freund, suffering from lung cancer, takes his own life.

1998

Centre for Addiction and Mental Health (CAMH) is formed from a merger of the Clarke Institute of Psychiatry, the Addiction Research Foundation and the Queen Street Mental Health Centre.

2000

Their bogus categories and made-up diseases are intended to subordinate, not to describe."

Lambda Literary Foundation nominated Bailey’s book as a finalist in the transgender category.

Christine Burns initiates an online petition which gathers thousands of signatures in just a few days, and the nomination is withdrawn

Jim Marks, Executive Director of Lambda Literary Foundation resigns.

J Michael Bailey steps down as Chair of Psychology at Northwestern.

Melanie Anne Philips apologized to customers of her voice instruction in that she now realizes that for many it cannot work: “out of all those who have sex reassignment surgery, only a very few have female minds. All the rest, no matter how feminine they have become, have male minds – they don't just think like men, then think as men”.

Philips has two children, and is gynephilic. She definitely avoids any mention of Autogynephilia.

2008

Susan Stryker. Transgender History. The trans academic makes no mention at all of the Blanchard binary and the upsets that it has caused among transsexuals. However Bailey’s book on the topic is quietly found in the book’s further reading section.

2010

Anne Vitale. Gendered Self: Further Commentary on the Transsexual Phenomenon. Trans therapist Anne Vital proposes a typology of three types with Gender Deprivation Anxiety Disorder (GEDAD) of whom the third approximates Blanchard’s Autogynephiles. She treats this deprivation, not a person’s gender identity. Many readers regard her typology as a humane improvement on Blanchard’s. The citation notes to her book include both Blanchard and Zucker, and the reading list on her website includes the Clarke Institute’s 1990 book, Clinical Management of Gender Identity Disorder in Children and Adults. However there is no mention of Blanchard in the text, and thus neither acceptance nor rejection of the inevitable notion that her 3 categories are a rewrite of Blanchard’s.

2012

Darryl Hill. Trans Toronto: An Oral History. Hill does have a short summary of autogynephilia, but refers to Blanchard only once in passing and then by his surname only; only his late 2005 paper is listed and Bailey is not mentioned at all.

2014

Dana/Thomas E. BevanThe Psychobiology of Transsexualism and Transgenderism: A New View Based on Scientific Evidence. Bevan, late transition, two wives, two daughters, writes: “it is clear that the concept of Autogynephilia is not well defined and cannot be easily operationalized. For this reason alone, it does not constitute a scientific theory”. However she writes as if Autogynephilia is being considered as a, or even the, cause of transsexuality rather than as a second type of transsexuality with a different etiology. She does not mention Michael Bailey’s 2003 book, The Man Who Would Be Queen. She does mention – actually she cites – Bailey with reference to twins, sexual orientation, and sibling order. But she totally ignores his book on Autogynephilia.

Will Rowe “Auditioning for Care” in Dan Irving & Rupert Raj (eds). Trans Activism in Canada: A Reader. This is the major discussion in the book of Toronto’s CAMH, the ground zero of autogynephilia. He writes of the "incredibly transphobic history of CAMH's GIC". He interviewed four trans men about their current experiences with the CAMH GIC. For an earlier period, the 1990s, he relies on Namaste's writings only. Namaste, as per her usual approach, never told us whether she was actually a patient at CAMH, although, as she lived in Ottawa and then Toronto while she transitioned, she quite likely was. No other trans women or other earlier patients are cited or discussed. It is particularly odd in Rowe's account that there is no mention of autogynephilia, and no mention of Blanchard.

2020

Barry Reay. Trans America: A Counter-History. Reay cites the title of Steiner’s 1985 book as an early usage of the term “Gender Dysphoria”, but says nothing at all about its contents. There is no mention at all of Autogynephilia, nor its proponents, nor of those who rallied against it. While this recent book continues until almost now, it also erases HBS and Cross-Dreaming.

Note also that the EN.Wikipedia article on CAMH says nothing at all about Steiner, Freund, Blanchard or Autogynephilia.

Olivia Jensen (1943 - ) earth sciences professor.

Oliver Jensen did his BSc, MSc and PhD at the University of British Columbia. He became professor in the Earth and Planetary Sciences department at the McGill University in Montréal. He also married and had two children.

After five years of careful consideration, Oliver transitioned to Olivia in 1989, through the Clarke Institute of Psychiatry (now CAMH) in Toronto.

She is still teaching at McGill.

Maxine Petersen (195?–) psychologist.

Lem Clemmensen, was a lecturer at the University of Toronto and Coordinator at the Clarke Institute (later the Centre for Addiction and Mental Health) Gender Identity Clinic, where he often was the one who was more sympathetic to the trans patients.  His 1986 masters thesis was Male gender dysphoria and criminality.

He was married to a woman, and they had two children. In 1991 Lem transitioned as Maxine, while still working at the Gender Identity Clinic where she was her own supervisor. Michael Bailey says that after transition she never saw her children again.

Maxine is quoted in Bailey’s The Man who Would be Queen as saying: “Most gender patients lie” and this one quote has been repeated extensively. She was part of the committee that revised the Standards of Care in 1998.

In 2000, she and Robert Dickey denied Synthia Kavanagh approval for SRS in that, being in prison, she could not do a Real Life Test. In the same year they were quoted in Vivian Namaste’s book defending the requirement that a trans person should do a year’s Real Life Test before starting hormones, and as critical of activists demanding hormones and surgery as a right. In September 2003 Maxine testified at an Ontario Human Rights Tribunal that government funding should be re-instated for SRS.

In November 2003 she resigned from HBIGDA (Harry Benjamin International Gender Dysphoria Association, now WPATH) when her boss Ray Blanchard did because it criticized Bailey’s book, an act which she described as ‘political correctness’.

She now lists her name as Maxine Petersen-Lee and offers private counselling.

I do not normally categorize persons discussed here as HSTS or autogynephilic, but as a loyal associate of Ray Blanchard, Petersen presumably endorses his binary, and as a father and ex-husband, that would make her Autogynephilic. She has not clarified whether she regards herself as such.

Unlike the Vancouver, Stanford, Charing Cross and Monash gender clinics, and unlike the Gender Recognition Panel, and the Free University in Amsterdam, the CAMH actually has a transsexual staff member.

In Vivian Namaste’s book, Petersen is not identified as trans, as per Namaste’s usual practice.  This makes her book an open text in that many of her readers know about Petersen from other sources.

Started-out-straight MTFs include crossdressers and most later-transitioning transsexuals, and our hallmarks are 1) We enjoy being women (and are deeply satisfied by anything that tells us that that’s what we’re doing). 2) We aren’t spontaneously and uncontrollably effeminate. 3) We’re attracted to women (whether we develop a fondness for men later on or not). 4) We gravitate toward business or technical careers. Started-out-gay MTFs include drag queens and most early-transitioning (say before 30) transsexuals. Their hallmarks are 1) They act like women and were effeminate from the start. 2) They don’t automatically love being women. If they’re well-received en femme, they seek it out.       Jennifer Usher and Diane Kearny, of the US Protestant HBS group, frequently use ‘autogynephile’ to mean non-HBS although they do not equate HBS and HSTS, and both HBS and HSTS are adamant that they are not the other.  "I do believe many today come from the behavioral mode of fetishism and through peer influence and/or as an extension of their own fetishism grow into what might well describe the autogynephilic not unlike the multitudes of the Lawrence's and Arune's in this world whom I suspect outnumber the true transsexual/HBS by an overwhelming number (Kearny-10/05/2009)".

                                              Dianna Boileau (1930 - 2014) secretary.

                                              A boy born in a Winnipeg home for unwed mothers was adopted by Mr Boileau, a forest ranger, and his wife, and named Clifford. They lived in Manitoba and then western Ontario. 

                                              In Fort Frances, Clifford consulted Dr Challis who realized that he was transsexual. Dr Challis had studied with Dr Robert Ernest Cowell in London. Dr Cowell was the father of Roberta

                                              Clifford acquired experience in office work by working for Dr Challis and a local Lawyer. At 17 Clifford, with his parents' approval, set out alone for Winnipeg. It was Dianna who got off the train. She stayed in a modest hotel and applied for secretarial work. However the police visited her because she had spent time in the lobby and talked to people. She confessed her male identity which quite confused the policemen. Her parents came to take her home. 

                                              Dr Challis explained the situation to them of having a daughter, and her parents moved to Port Arthur (now part of Thunder Bay) so that Dianna could start her new life. However her mother was uncomfortable in the big city of 30,000, and they went back to Fort Frances. 

                                              Dianna moved to Calgary. She obtained work as a stenographer and art-school model. She developed a friendship with Rosemary, a divorced elevator operator. She and Rosemary shared a home. After getting too close to a man, and not wanting him to find out her sex, Dianna persuaded Rosemary to move to Edmonton.
                                              She got a job as an instructor in a dance school. She got drunk for the first time with one of the other instructors, and they were arrested for petty vandalism. When her maleness came out she was sent to a mental hospital for an evaluation. The hospital outed her to Rosemary, but it made no difference. Dianna and Rosemary moved to Toronto. Dianna found employment as a stenographer in a law firm. 

                                              On a stifling June night in 1962, Dianna and Rosemary went for a midnight car ride to cool down, were sideswiped, and Rosemary died after the car hit the guardrail. Dianna's boss, being a lawyer, took charge. She told him that she was a transsexual. On the third day, she returned to work, and was arrested that afternoon, charged with criminal negligence causing death and dangerous driving. She was read by the prison matron, and transferred to men's facilities. She spent 4 days in the Don Jail for men, until her boss raised the bail amount. Her case also became a media sensation. She and her boss were bickering under the stress, and they mutually agreed that she should get another job. Dianna was arraigned as both Clifford and Dianna but was allowed to dress as Dianna. She was acquitted of criminal negligence at a trial in September. At the trial for dangerous driving in February, after 10 minutes the all-male jury also returned a verdict of not guilty. 

                                              Dianna started drinking, lost her job and used up all her savings, and attempted suicide. The attempt outed her at hospital, and the police informed her landlady. However the landlady took her in hand, stopped her drinking, and pushed her to get a new job. 

                                              She met other transsexuals, obtained leaflets from the Erickson Educational Foundation, and phoned Dr Leo Wollman in New York, which resulted in a prescription for female hormones from a local doctor. She started electrolysis. In 1969, she and a friend called Bambi, went to New York to be castrated by a doctor in Yonkers. They took four days to recover from this. In August she approached the gynecological department of Toronto General Hospital and was referred to Dr Betty Steiner at the new gender identity clinic that was being formed at the Clarke Institute of Psychiatry

                                              She underwent several months of evaluation, which included two weeks as an inpatient at the Clarke Institute, and getting a local social worker to interview her parents. She was sent to the penile plethysmograph, but was allowed to exit in disgust. Dianna became the first transsexual patient to be approved for surgery by the GIC, the first to be financed by the Ontario Health Insurance Program.  Was hers the first transsexual surgery in Canada?  There were some others at about the same time at McGill University and one at Dalhousie University. 

                                              Her operation was on 20 April 1970 at the Toronto General Hospital. The Edgerton technique devised at Johns Hopkins was used. Dr Steiner gave her a kiss as she went under the anaesthetic. Dianna was kept in the hospital for three months before being discharged. She gave a press conference in mid-September, giving only her first name. However she was overshadowed by Leo Wollman who flew up for the event, and predicted that transsexual women would be able to become pregnant within 10 years. 

                                              Dianna wrote her autobiography in 1972, dedicated to Betty Steiner, with an Introduction by Leo Wollman, and has stayed out of the news ever since.

                                              Dianna seems to have been a bit naughty and practiced time compression in her autobiography.  She states that in 1962, the time of the trial, she was 15 years younger than Rosemary who was about to turn 40, which would have made her 24.  In addition the book leaves the impression that she was one year in Calgary and less in Edmonton, and my mental image while reading it was that she was in her early twenties when the car accident happened. However the Globe and Mail articles on her trial state that Dianna was 32 and Rosemary 45.  Hence Dianna must have been born in 1930, and will be 80 now.

                                              With the revised chronology, Clifford was a patient of Dr Challis in 1946-7.  So the statement, that he knew about transsexualism in that he had studied with Dr Robert Ernest Cowell, is odd in that Roberta had not started her transition then.

                                              Dr Wollman is an unreliable narrator as we know from his movie, Let Me Die a Woman, and his prediction of organ transplants so that trans women can have babies, is the same nonsense that he spouts in the 1978 film.  Ten years from 1970 is 1980.  We are 30 years further on and it is still not likely.

                                              If the Clarke GIC had continued the personal service that it gave to Dianna, it would have a much better reputation.

                                              Betty Steiner (1920 - 1994) psychiatrist.

                                              Betty married Andrew Conduit who worked for Reader's Digest from 1948-1979, eventually rising to Senior Vice president, and they lived in the upscale Castle Frank area of Toronto.

                                              Dr Steiner, MB, FRCP (C) was a general practitioner for several years, and then retrained as a psychiatrist. She was appointed psychiatrist at the Women's College Hospital, where she was included in a 1963 Globe and Mail article on women in the professions. In 1969 she became the first head of the newly created Gender Identity Clinic at the Clarke Institute of Psychiatry. "I wanted to do some clinical research, and as I knew nothing about transsexualism it seemed an interesting field to explore (Steiner, 1985: xi)".

                                              Later the same year she personally guided the Clinic's first client approved for surgery, Dianna Boileau, through the process, even giving her a kiss as she went under the anaesthetic. Dianna dedicated her autobiography to Dr Steiner.

                                              At the January 1973 meeting of the Ontario Psychiatric Association, Dr Steiner reported that six patients had been operated on, five of them in the previous six months.

                                              In 1981 she examined inmate Katherine Johnson and recommended female hormones but was ignored. She did not see Katherine again until 1989. In 1982 Dr Steiner and Dr Hucker wrote a report for Correctional Services of Canada that recommended that gender variant inmates be 'frozen' at the stage of feminization or masculinization that they were at when incarcerated. They were to be placed in accordance with their 'anatomical structures'. Hormone treatment but not sex-reassignment surgery could be provided.

                                              In The Globe and Mail in 1983, Dr Steiner reported that "between 1975 and 1983, the clinic had 111 surgical patients, of which 59 were biological males wishing to become females and 52 females wishing to become men" and "women who become men are much more stable as a group than their counterparts". In 1984 in the Toronto Star she attributed the social success of the 102 clients who had had surgical sex change through the Clinic in its first 15 years to the fact that only 1 in 10 'men' who request it are approved.

                                              Through the mid-1980s she apparently got on well with Susan Huxford, the leader of the peer-support group FACT, perhaps because they were both women of a similar attitude and of almost the same age. She was delighted to get gossip from Huxford re the activities of the transsexuals in Toronto and Hamilton. At the same time patients at the GIC were complaining more of how they were treated. Certainly the special treatment that Dianna Boileau had been given was not continued.

                                              In 1985 she edited the anthology Gender Dysphoria wherein the foundations of the Autogynephilia-HSTS binary are clearly laid out. She has her name with others on several of the foundation texts that had been published elsewhere, but in the essays in the book authored by herself alone the ideology is muted, although she still divides male-born transsexuals into homosexual and heterosexual (based on their birth body), insists that all female-born transsexuals are homosexual (i.e. gynephilic) and that only male-born persons are ever fetishistic. She regards a trans woman finding a husband as ‘unusual’. She feels that many transsexuals have poor impulse control. She was at first a follower of Robert Stoller, but having Kurt Freund and Ray Blanchard on her staff, it is no surprise that she drifted to their conceptualizations. She echoes Stoller's claim that there have been only three female transvestites. She speculated that transsexual women (i.e trans men) differ from lesbians because of a "sustaining influence of the high level of aggression within the family of the' transsexual-to be' (Steiner, 1985: 420)".

                                              She retired as head of the clinic in 1986 and was replaced by Robert Dickey. However she co-edited a further anthology on the work of the GIC in 1990, and contributed a paper on Intake Assessment.

                                              In 1994 Mr & Mrs Conduit were overcome by fumes from a car that had been left running in the garage of their home. Steiner died at the time, and her husband 10 days later.

                                              Ray Blanchard (1945 - ) psychologist.

                                              Ray Blanchard’s father, an aviation metal smith died in the Japanese attack on the USS Franklin, just before his namesake son was born. The son was raised a Catholic in Hammonton, New Jersey.

                                              He did an A.B. in psychology at the University of Pennsylvania, 1967, and PhD in psychology at Urbana University, Illinois, 1973, the year that the American Psychiatric Association removed homosexuality from its list of diseases. He had a draft classification of 4-A, the sole surviving son of a service member killed in action, and so was exempt unless war was officially declared, which the US never did against Vietnam.

                                              He emigrated to Canada anyway. He did post-doctoral research at Dalhousie University investigating learning processes in animals (mainly rats) until 1976 when he became a clinical psychologist at the Ontario Correctional Institute in Brampton (now the Brampton Adult Training Centre). There he met Kurt Freund while working with sex offenders. They made plans to collaborate, and in 1980, Blanchard started working at the Clarke Institute in Toronto (now the Centre for Addiction and Mental Health).

                                              He did work on male birth order and sexuality and claims that the number of older brothers increases the chance of homosexuality in right-handed men, but not in left-handed men or in women at all. He claims that pedophiles have lower than expected IQs and higher rates of non-right-handedness. He hypothesizes that “these and other correlates of pedophilia reflect some perturbation of prenatal neurodevelopment in subsequently pedophilic males”.

                                              A lot of his work utilizes phallometric diagnosis. He is best known for his re-presentation of Kurt Freund’s ideas on fetishistic transsexualism which he re-labelled as ‘autogynephylia’. He insisted for many years that there are no gay trans men, and consistently ignored Lou Sullivan’s letters in the 1980s explaining that Lou and many others are indeed gay trans men. Further to this he does not acknowledge autoandrophilia, which is the term that his theoretical system should apply to such trans men. However he now claims to have published two of the earliest reports of gay trans men (but without using that terminology).  He also insists and persists in referring to heterosexual trans women as homosexual, and refuses to use the word ‘androphilic’.

                                              Blanchard is a Professor of Psychiatry at the University of Toronto (despite being a psychologist not a psychiatrist), and in 1995, after Kurt Freund’s retirement, he became Head of Clinical Sexology Services in the Law and Mental Health Programme of the Centre for Addiction and Mental Health (CAMH). He served on the gender dysphoria sub-working group for the DSM-IV, 1994, and is serving as Chair of the paraphilia sub-working group for the new DSM-V. He is an Associate Editor of Sexual Abuse: A Journal of Research and Treatment and a member of the Editorial Board of the Archives of Sexual Behavior.

                                              He supported Michelle Josef’s lawsuit in 1999 to get gender surgery relisted under the Ontario Health Insurance Plan. He supplied material to Lisanne Anderson for her Autogynephilia Site.

                                              He gave a paper at the Third International Body Integrity Disorder Meeting in 2003 comparing BIID (apotemnophilia=desire to have a limb removed) to Gender identity disorder, as did Anne Lawrence. This has fed the usage on the religious right of referring to sex changes as sexual-amputation fetishism.

                                              Michael Bailey’s The Man Who Would Be Queen, 2003, recycles many of Blanchard’s ideas. Blanchard's favourable review of the book is the only review that he ever wrote for Amazon, and he resigned from HBIGDA (Harry Benjamin International Gender Dysphoria Association, now WPATH) when it criticized the book, an act which he called 'appalling'.

                                              In 2002 the CAMH GIC was awarded a Presidential Citation from Div 44 of the American Psychological Association.  Ray Blanchard accepted the award on behalf of CAMH.

                                              The population of Ontario being over 13 million, there should be around 200 trans persons completing transition each year.   Before the suspension of Ontario Health Insurance Plan (OHIP) funding for gender surgery in 1998 Blanchard's management had kept the number approved to around 10 per year, and it looks as if the numbers approved since the re-instatement in 2008 will be approximately the same.

                                              As Blanchard manages a clinic that rejects most of its applicants, and proclaims a universal theory based on the minority that it accepts, it is a reasonable assumption that the majority are rejected precisely because they do not fit the theory.  For this crime of contradicting the theory, they are denied OHIP funding.

                                              In 2004 he wrote: “This is not waving a magic wand and a man becomes a woman and vice versa.  It's something that has to be taken very seriously. A man without a penis has certain disadvantages in this world, and this is in reality what you're creating" (Jane Armstrong. “The Body within, the body without”.                                               I am not discussing the concept of autogynephilia in any detail here as I have already done so here.

                                              As his insistence on calling heterosexual trans women ‘homosexual’ shows, Blanchard regards trans women as men, and trans men as women.  This was shown again in the 2004 article in The Globe and Mail.  Blanchard has tried to claim that he is not transphobic, but until he changes in his regard of which gender we are, it is inevitable that many, probably most, of us do regard him as transphobic.  This attitude will probably not change until the next generation take over the gender clinics.   It does appear that Blanchard and Zucker are attempting to solidify their position in the DSM-V, so that it will last for another generation.

                                              Of course Blanchard supported Michelle Josef’s lawsuit in 1999 to get gender surgery relisted.  The CAMH GIC is a stronger institution when it can offer the carrot of paid-for gender surgery on the Ontario Health Insurance Plan.

                                              Blanchard claims that pedophiles suffered “perturbation of prenatal neurodevelopment”.  The HBS people claim that the same thing happened to them, but don’t phrase it so well.

                                              Unlike Kurt Freund who was married to a woman and had had children, and unlike James Cantor who is openly gay, Blanchard has been very coy about his private life.  Andrea James’ article of November 2009 is by far the best on this particular topic, but I would maintain that his closetry denies him the right to claim to be gay.

                                              Being a right-handed eldest child with a heterosexual younger brother, I am not sold on the birth-order effect.

                                              Blanchard, Zucker and Cantor are all by upbringing American.  That is a bit too many to be all in one Canadian clinic, especially the dominant Canadian clinic on transsexuality.
